The swelling is due to lymphatic edema distal to the constriction.

Peau d'orange is caused by cutaneous lymphatic edema.

Lymphatic edema can be developed in many acral cases accompanied by a thickening of the folds of the skin, hyperkeratosis and papillomatosis.

The result of this is the development of chronic lymphatic oedema, due to accumulation of fluid, and eventually to elephantiasis, which is irreversible enlargement.
